COMMUNITY LIVING SERVICES DIVISION JOB DESCRIPTION DIVISION SUPPORT SPECIALIST, SUPPORTED LIVING JOB SUMMARY: The Support Coordinator (SC) for Supported Living is responsible for the day to day management of staff and services for those assigned individuals being served in the SL section of the Community Living Services Division. He/she will be directly responsible to the assigned Division Manager. The SC will provide direction, supervision, and training to the assigned teams and will be responsible for the scheduling and oversight of direct support staff. Each Program Manager will act in the role of “on-site manager” for the CLS division on a rotating basis. On those days, he/she will be scheduled to work in the office. JOB DUTIES: 1. Oversee staffing for all teams assigned to the division. a. Hire, train, reprimand, develop, discipline, and, when necessary, terminate direct support employees. b. Ensure staffing supports are provided as approved in the Plan of Care. 2. Work with Division Manager and Administrative Assistant to monitor billing and payroll for each team on a regular basis. 3. Provide on-site consultation and training in each home/site assigned and provide documentation for each session. 4. Provide oversight for all teams to ensure the provision of quality services. This may include: a. Review of health and safety concerns for each individual served b. Review of quality of life concerns as outlined in the individual plan c. Review progress and conduct training concerning ISP development for each individual served. d. Review of scheduled activities as related to the ISP e. Review and correct documentation to ensure accountability to licensing agencies and ASN policies. f. Provide all necessary on-going training with staff (CSPs 1-4). 5. Complete other duties as assigned. a. Attend meetings and in-service training sessions as needed b. Work with supervisors and staff to develop annual goals c. Serve as “on-call” supervisor on a rotating basis. d. Attend national, state, and local conferences as requested to keep abreast of “best practices” in the field. KNOWLEDGE EDUCATION SKILLS The Support Coodinator (SC) will have a minimum of a Bachelor’s degree in a Social Work or Human Service field of study or a minimum of two years of college with a minimum of two years of direct service delivery for individuals with developmental disabilities. The SC will have excellent communication and written skills. The SC must possess a driver’s license and insurance and must be willing to travel, as needed. The SC must organize her/his time well and must be able to complete tasks within designated time lines. He/she must be able to express himself/herself clearly and to be a strong advocate for quality services for the individuals served. He/she must have excellent leadership skills. A preference will be given to individuals/family members with a disability when all other qualifications are equal. DAYS/HOURS: This position is a twelve-month salaried position. The position is a full time position. Hours are flexible, based on the needs of the individuals served and approved by the Division Manager. Evening and weekend work is required to meet the requirements of the position.
