Join our New Hampshire Detox team - now powered by TruHealing as a Discharge Planner! Tuesday through Saturday, the Discharge Planner will manage treatment services including ongoing coordination of care, program engagement, advocacy, behavior intervention, LOC transitions, aftercare, follow-up, and discharge planning.

Requirements:

  • Preferred college degree from accredited university or equivalent.
  • Preferred 2+ year of experience working in behavioral health.
  • Preferred bachelor level SUD certification (LADC Equivalent) or CRSW/CADC.
  • Knowledge of addictions and mental health complications.
  • Knowledge of community health resources that support life improvement in physical, emotional, mental, and spiritual health, economic security, employment, positive social and family interdependence and positive cultural identity
  • Knowledge of emergency procedures used in case of alcohol and/or drug overdose.
  • Knowledge of the stages of alcohol and other drug withdrawals and ability to take appropriate action at each stage.
  • Knowledge of Federal, State and Municipal laws and/or regulations that regulate the treatment of alcoholism and drug addiction
  • Flexibility to adapt to schedule changes and assumption of responsibilities not delineated in the job description which are related to work as a member of an addiction treatment team

Responsibilities:

  • Upon patient's arrival and admission, conduct a case management interview with patient to determine if there is a need for FMLA, Short Term Disability Insurance and if there are any pending legal issues. Will also obtain ROI for referent, family, EAP and/or employer. Ensure patient has notified family of arrival. (Does not apply if your facility has a Case Manager)
  • Produce all documentation needed for the patient's employer, schools, legal, etc. including, but limited to, FMLA, STD, LTD, Treatment Letters, etc. (Does not apply if your facility has a Case Manager)
  • Meet with each patient once weekly, at a minimum, for Discharge Planning.
  • Complete patients aftercare planning, which includes:
    • Develop aftercare plan with patients
    • Research aftercare treatment options for patients
    • Locate transitional housing if needed
    • Locate AA/NA meetings, specialists, doctors, therapists, psychologists, and MAT providers in patients' home area
    • Obtain required Return to Work signatures for patient
    • Fax or email discharge summaries and other treatment paperwork to aftercare resources
    • Contact family and Business Development or referent to review aftercare plan
  • Assist client with completion of exit survey
  • Develop a strong, ongoing relationship with patients and their families. Maintain ongoing healthy communication with the patient's families.
  • Accurately share information in team meetings.
  • Ensure medical records are sent to the facility where a patient is discharging to.
  • Facilitate referrals to ancillary services. Ensure the correct documentation is collected and/or received.
  • Facilitate SUD and Mental Health groups that are assigned.
  • Assist in ACA/AMA blocking with the Clinical and Operations department.
  • Maintain a basic understanding of health insurance benefits, health insurance terms, and other payment methods that are used for our organization.
  • Report to the Clinical Director or Director of Clinical Programming for all emergencies and/or concerns
  • Remind patients of the program rules and regulations as necessary and report infractions appropriately.
  • Complete all assigned documentation in a neat, accurate and timely manner.
  • Complete incident reports according to policy in a timely, accurate manner.
  • Exercise sound professional judgment and seek assistance as necessary to effectively manage patient behavior.
  • Protect the privacy of all patient information in accordance with the organization's privacy policies, procedures and practices, as required by federal and state law, and in accordance with general principle of professionalism as a health care provider.
