GENERAL SUMMARY:
Responsible for conducting assessments of clients entering Faith Mission FC shelter and programs. Ability to assess for internal and environmental strengths and resources, behavioral health needs including AOD level of care, development of initial treatment plan and coordination and linkage with appropriate case managers and intervention groups. Provide supervision and development of supportive services staff. Ensure that services provided are within the guidelines of agency mission and vision statement and comply with grant, funding source and contractual parameters. Responsible for overseeing clinical/supportive services and collaborative relationships for Faith Mission of Fairfield County including placement of social work interns, and delivery of supportive services that result in successful outcomes for Faith Mission residents.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Plan, develop, and implement comprehensive supportive services to benefit clients of Faith Mission FC:
- Develop and implement client care systems and methodologies that are client centered and individualized into the delivery of services
- Facilitate and cultivate collaborations that provide onsite specialized supportive services such as legal assistance, access to benefits, medical care, short term intervention, etc. to persons residing in shelter
- Create and maintain Individual Work Plans for direct reports that are consistent with agency’s Continuous Quality Improvement plans
- Work with Director and Assistant Director of Operations
- Work with Leadership Team to implement Best Practices in Social Services delivery
- Work closely with the Assistant Director to oversee the delivery of individualized “wrap around” services that address the holistic needs of clients, including housing, employment, healthcare, mental health, spiritual support, and transportation, etc.:
- Participates in the support and implementation of effective and quality behavioral health and case management services to be provided to the clients of Faith Mission FC.
- Provide formal, individual supervision as appropriate for LSS/FMFC program staff that will ensure professional development and provide orientation, training, and coaching that will enhance the ethical, confidential, and respectful delivery of services to clients.
- Possess the ability to conduct comprehensive multiaxial diagnostic assessments with assigned and appropriate clients entering Faith Mission FC emergency shelter and programs.
- Determine clinical needs including AOD level of care
- Coordinate and link clients with appropriate case manager and intervention groups
- Develop initial treatment plan based on diagnosis, strengths and needs
- Provide short term intervention and/or groups as indicated by clinical assessment/treatment plan
- Ensure linkage and referral to a vast array of community resources that will provide long-term support for individuals once they leave the program
- Implement system for documenting and responding swiftly to client concerns in a fair and consistent manner
- Ensure prompt and accurate completion of required documentation including HMIS data, progress notes, records of services received, housing outcome information, releases of information, consent forms, and other important client information in case files:
- Work closely with Assistant Director to ensure prompt and accurate entry of data into the HMIS database
- Work closely with Assistant Director to monitor performance outcomes and compliance with funder-required program and administrative standards
- Respond swiftly to identify and correct performance problems to ensure ongoing compliance with grants
- Prepare for and participate in auditing visits from funding and oversight entities to ensure compliance with applicable standards
- Work closely with Faith Mission FC Leadership team to ensure effective system for measuring and reporting achievement of program outcomes and “productivity” of supportive services staff
- Supervise direct reports to provide informed, professional services to clients and supportive services staff:
- Provide weekly individual supervision to direct reports to ensure the delivery of quality customer service
- Work with Direct Reports to ensure weekly clinical case reviews with key staff to ensure positive outcomes for all clients
- Provide prompt feedback and coaching to direct reports
- Work with Leadership Team to hire, evaluate, develop, and dismiss staff according to LSS/FMFC policies and procedures
- Maintain independent licensure in order to provide quality field instruction to student interns
- Assist with crisis intervention and de-escalation
- Provide specialized training to all Faith Mission FC staff. Provide other clinically oriented trainings to Faith Mission FC staff and others as needed and required.
- Provide professional leadership and direction to clinical/supportive services staff:
- Supervise staff in a manner that promotes the guiding principles of LSS/FMFC
- Understand and promote a culture that values diversity and celebrates the positive aspects of a multicultural society
- Model professionalism and problem-solving behavior for clients, staff, and community by maintaining a calm, professional demeanor at all times
- Perform as an invested member of the LSS/FMFC management team:
- Attend all Management Meetings and fully participate in agency decision making
- Participate in biweekly Management Team, all-staff, and other meetings on a regular basis
- Participate in community planning efforts and other meetings as directed
- Support the mission, policies, and procedures of LSS/FMFC
- Provide on-call support on a clinical basis
- Perform other duties as assigned
QUALIFICATIONS
EDUCATION:
Master’s degree in Social Work or related field and LISW or LPCC required
EXPERIENCE:
- Experience working with homelessness, mental health, and/or chemical dependency required
- Three years supervisory experience required

- Ability to effectively communicate with highly diverse populations
- Proficiency in Microsoft office suite required
- Commitment and aptitude for working in a team environment
- Ability to lift 25 lbs and climb stairs
OTHER REQUIREMENTS:
- Able to work a flexible schedule to include evenings and weekends as needed
- Valid Ohio driver’s license, access to reliable insured transportation and ability to meet LSS insurance standards required
