Under general direction and following established medically-approved policies and procedures, the Diabetes Nurse Educator (DNE) will provide diabetes expertise to Ventura County Medical Center/ Santa Paula Hospital staff, providers, patients, and families. The DNE is responsible for the assessment, planning, implementation, coordination, and evaluation of the Diabetes Education Program and functions in the roles of teacher and consultant. This position works in collaboration with physicians and other interdisciplinary team members to address the care of patients identified as at risk for hypoglycemia, hyperglycemia, and related conditions. Distinguishing Characteristics: The DNE will serve as a patient advocate and resource for hospital staff, educate patients during their hospitalization, and provide diabetes education and recommendations upon transition out of the hospital. Opportunities to improve current diabetes care of each unit will be identified, and improvements addressed with interdisciplinary collaboration. The DNE utilizes evidence to drive clinical decisions and practice related to individual and population health. The DNE establishes goals and strategies for meeting care coordination needs across settings with the patient, family, and/ or other care providers. The DNE engages in lifelong learning and serves as a role model of professionalism, as well as, participates in continuous quality improvement initiatives and assumes responsibility for their professional development. The DNE will provide education and support in collaboration with the Nursing Education department.PAYROLL TITLE: Senior Registered Nurse - HospitalNew Hire IncentiveMay be approved up to 10% of a one-time New Hire Incentive pending Board approval.CERTIFICATION PAY: Incumbents in this classification are eligible for certification pay at a rate of $0.813 per hour based on his or her scheduled work week hours and not to exceed the scheduled work week hours for each qualified certification possessed up to a maximum of five (5) certifications.EDUCATIONAL INCENTIVE: Some positions may be eligible for educational incentive. This incentive may be 2.5%, 3.5%, or 5% for incumbents in eligible positions based on completion of an Associate's, Bachelor's, or Master's degree that is not required for the classification. BILINGUAL INCENTIVE: Some positions may be eligible for bilingual incentive depending on the applicable memorandum of agreement and the needs of the department. In order to qualify for this incentive, incumbents in eligible positions must take and pass the applicable bilingual fluency examination. AGENCY/DEPARTMENT: Health Care Agency – Ventura Medical CenterSr. Registered Nurse-Hospital is represented by the California Nurses' Association (CNA) and is eligible for overtime compensation. NOTE: Salary placement will be determined according to the current CNA memorandum of agreement.The eligible list established from this recruitment may be used to fill current and future Regular (including Temporary and Fixed-term), Intermittent, and Extra Help vacancies for this position only. EXAMPLE OF DUTIES
Duties include but are not limited to the following: Implements nursing processes (assessment, planning, implementation of care plan, evaluation of patient response); revises plans as needed;Administers basic and advanced competency nursing care in accordance with VCMC/SPH policies and procedures and Patient Bill of Rights;Provides direction to others regarding complex health and patient problems;Applies current research and evidence-based care to practice;Provides patient and staff education including the coordination of educational programs;Participates in and collaborates with hospital, nursing administration care committee; community partners, and/or diabetes care team.Provides quality diabetes self-management education, medical nutrition therapy and diabetic wound management based on assessed needs. Utilizes appropriate teaching techniques that are sensitive to the learning preferences of the person with diabetes;Reviews, identifies, reports, and corrects deficiencies in practices, products, or equipment as necessary;Documents all individual contacts/ visits in the Electronic Health Record;Develops, implements and documents performance and/or quality improvement processes;Recognizes problem situations and takes appropriate steps leading to correction.Coordinates patient care conferences;Serves as a preceptor for new graduates, re-entry nurses, etc; andPerforms other related duties as required.
MINIMUM QUALIFICATIONS
These are entrance requirements to the exam process and assure neither continuance in the process nor placement on an eligible list. DEPARTMENT/AGENCY SERVICE: Currently working as a Regular employee for the County of Ventura Health Care Agency (HCA). NOTE: A Regular employee is an employee who holds an allocated full-time or part-time position in the County budget excluding (but not limited to) extra help or intermittent employees, provisional appointees, enrollees in training programs, and/or independent contractors. EDUCATION, TRAINING, and EXPERIENCE: Requires two (2) years of full-time professional registered nursing experience including two (2) years of experience coordinating educational programs for patients and/or staff.NECESSARY SPECIAL REQUIREMENTSMust possess and maintain a current, valid license as a Registered Nurse issued by the State of CaliforniaMust have a current, valid Basic Life Support (BLS/ CPR) certification by the first day of employmentCertified Diabetes Care and Education Specialist (CDCES) or Board Certified- Advanced Diabetes Management) BC-ADM credential preferred. If not already DCES or BC-ADM will sit for the certification examination within 24 months of hire dateDiabetic Wound Certified (DWC) preferred. If not already DWC will complete a Diabetic Skin and Wound Management Course within 24 months of hire dateOTHER REQUIREMENTS: This position may be subject to State Health Officer Orders regarding vaccine verification and/or testing. These requirements are a condition of employment.
