Community Support Specialist

Position Summary
The Community Support Specialist provides individuals and families with the services and resources necessary to promote recovery, rehabilitation and resiliency based on goals identified in the individualized service plan.
Essential Job Duties- Build and maintain relationships with community stakeholders,
- Establish and maintain resources and information relating to medical services/programs (e.g., Medicaid) and job opportunities,
- Actively participate in the Multidisciplinary Treatment Team (MDT),
- Complete a risk and needs assessment on youth as they enter the Program, and complete re-assessments within established contractual guidelines,
- Evaluate, develop and document an individual service/care plan (ISP) for the youth based on the assessments,
- Ensure youth receive applicable assessments within the required timeframe of enrollment and that this data is used in the development of the ISP,
- Participate in building youth’s ability to function productively in interpersonal relationships, community coping activities and functional skills (e.g. Socialization skills, developmental issues, daily living skills, school and work readiness activities and education in co-occurring illness),
- Attend and represent the program at court hearings, meetings and youths’ legal proceedings as needed,
- A minimum of 60% of direct client services shall be conducted face-to-face and in community locations,
- Partner with Therapist to coordinate needs assessment and additional activities related to ISP,
- Ensure each youth has post assessments completed and has a transitional plan before release date,
- Participate in monthly audits to ensure all pertinent information is recorded,
- Maintain service notes daily,
- Schedule off-site appointments and transportation for youth,
- Conduct home visits with youth’s families in accordance with Program and state requirements,
- Provide individualized interventions to encourage the development of natural supports in community, workplace and school environments,
- Operate vehicles as required in accordance with the transportation policy,
- Attend required training/professional development events and maintain appropriate certification(s) and license(s),
- Attend and maintain CPR and First Aid certification by nationally recognized organization,
- Attend and maintain appropriate crisis intervention and physical restraint training and certification as defined by state and contract requirements,
- Maintain appropriate certification(s) and license(s),
- Assist with special projects and other duties as assigned.
Required Education, Training and Experience- Bachelor’s degree in Human Services or related field,
- Two (2) years of relevant experience with target population,
- Strong collaboration and problem-solving skills,
- Must possess and maintain valid driver’s license in the state of employment and have a driving record that meets the standards of the AMIkids Transportation Policy,
