The Community Outreach Specialist (COS) is part of the BMS Population Health Outreach team. The COS performs processes necessary for optimizing Value Based Purchase and HEDIS outcomes for the organization and reducing unnecessary Emergency Room visits. The COS make outgoing calls, register patients, schedule appointments, and messaging in compliance with organizational scheduling guidelines, processes, and policies based on the lists of targeted services. The Community Outreach Specialist actively participates in assigned performance improvement teams and work towards optimizing BMS policies, processes, and systems to ensure that set goals and targets are achieved. Assisting in the coordination and implementation of evidence based programming as well as wellness events to improve health outcomes for patients is vital responsibilities of the COS. The COS exhibits and promotes a customer service standard of excellence in performance of all duties with patients, coworkers, and outside contacts. Salary commensurate with experience.


Schedule/Hours:

Mon-Fri; 8:00-4:30 PM, some weekends


Essential Functions

  • Assure guidelines in scripts for VBP/HEDIS calls and patient demographics are accurately verified and documented according to procedures.
  • Create schedules and effectively communicates BMS' scheduling policies to patients.
  • In Review emergency room visits reports to reach out to BMS patients and schedule them with their respective doctor.
  • Develops monthly reports on outreach efforts. Report findings to managers.
  • Maintains VBP lists to assist the organization in improving overall provider utilization rates.
  • High School Diploma
  • 3+ years of recent experience in a medical office
  • 3+ years of recent experience reviewing and/or documenting in patient medical records.
