Community Health Worker (Maternal Infant Child Health) Details

Position Summary:The Community Health Worker (CHW) will join our Maternal/Infant Health Care team to support the community by addressing maternal and infant health challenges. Based in the Bronx, the CHW promotes women's health before, during and after pregnancy. The CHW maintains a caseload of 25 individuals and provides referrals to various support services such as prenatal care, WIC, doula support, immunization, breastfeeding, and family planning. This is currently a remote (work from home) role, however candidate must be comfortable with the prospect of returning to onsite duties (including home visits) in the Bronx in the future. Minimum Qualifications:

  • Associates Degree (or High School Diploma with some college credits) in Public Health or related field of study, required.
  • True passion and dedication to the health of mothers and infants.
  • Ability to conduct outreach to at-risk women (pregnant and parenting)
  • Previous experience in outreach, referrals, or women’s health education- highly desired.
  • Bilingual in English and French or Spanish is a huge plus.
Key Duties:
  • Conducts extensive outreach to identify, engage, and enroll women of childbearing age into the program.
  • Conducts regular home visits and service coordination.
  • Conducts health-related workshops in the office and home-based one-on-one health education (prenatal care, immunization, breastfeeding, family planning).
  • Provides women with referrals to various supports, services, entitlements, and benefits. Escorts to appointments, when necessary.
  • Escorts patients to various appointments.
  • Enters client data into software on a daily basis.
  • Assists with special projects as needed.
