- Reporting to the Coding Operations Manager
- Completes all work in accordance with Hospital-defined productivity and quality standards. Utilizes encoder software applications (including online references) in the assignment of ICD-9-CM codes and CPT-4®/HCPCS procedure codes for appropriate APG assignment. Utilizes coding guidelines established by the Centers for Medicare/Medicaid Services (CMS), American Hospital Association (AHA), and Unified Revenue Organization/Holy Cross Hospital (URO/HCH).
Responsibilities:
- Assures accurate and complete assignment of codes by analyzing physician/provider documentation contained in assigned ambulatory surgery, interventional radiology, labor and delivery, observation, and pain management outpatient health records to determine the first listed and all secondary diagnoses and procedures.
- Any one of the following certifications through AAPC or AHIMA: RHIA, RHIT, CCS, CPC, CPC-H,
- Navigates the electronic patient health record and other computer systems in determination of diagnoses and procedures to be coded.
- Validates charges, when necessary, by comparing charges with health record documentation.
- Tracks issues (such as missing documentation or charges) that require follow-up to facilitate coding in a timely fashion.
- Communicates effectively with clinical staff, physicians and office staff regarding documentation.
- Maintains up-to-date knowledge of changes in coding guidelines and regulations. Participates in educational opportunities to enhance knowledge in coding and reimbursement systems. Maintains certification to validate coding skills.
What you will need:
- High school diploma or equivalent is required. An Associate’s Degree in Health Information Management or a related field is preferred with 3-4 years of hospital outpatient coding experience.
- Must possess a comprehensive knowledge of medical terminology, anatomy and physiology as well as basic disease processes, and basic diagnostic and procedural coding, as normally obtained through a coding certificate program.
- Ability to utilize both manual and automated versions of the ICD-9-CM and CPT4 coding classification systems.
- Detail oriented and high accuracy in data entry. Well-developed communication skills, both written and oral. Able to communicate effectively with individuals and groups representing diverse perspectives.
- Ability to work with minimal supervision and exercise independent judgment
- Must be comfortable functioning in a collaborative, shared leadership environment.
- Abides by confidentiality requirements as they relate to the release of individual or aggregate patient information.
