  • CNA/CMT/HHA License/Certification (Companions Don't Need This)
  • Annual Physical (Doctor stating you are well and have no limitations to work)
  • 2 Step PPD/Chest X-Ray (Chest X-Ray is a One-Time ONLY event once the initial chest-ray is completed there is a annual TB Clearance Required)
  • Criminal Background Check (If you don’t have a Qualified Criminal background check complete we can supply you with the form and the location to get it done)
  • CPR
  • License/State ID
  • Social Security Card
  • Passport, Green Card, or Work Permit
