ResponsibilitiesAs a Hospice Clinical Supervisor, you will manage the coordination, supervision, and implementation of professional and supportive services to clients in a safe, cost-effective manner. You will also provide education and training related to clinical practice issues and regulation and reimbursement changes with new employee during precepting phase of new hire orientation. In this role you will be responsible for:
  • Planning, directing and participating in delivery of professional services to hospice clients
  • Communicating significant findings, problems, changes in condition or environment to the physician and other personnel involved with patient care
  • Initiating plan for patient safety, using the patient, family and community resources
  • Identifying deviations from normal nursing and normal patient condition status
  • Directing all aspects of operations and care delivery in accordance with the mission and values of Careline Health Group
  • Supervising the coordination of services with providers, vendors or facilities in the delivery of care and related issues
  • Processing and coordinating patient referrals, physician orders, staff assignments and patient re-certifications, and communicating between the referring physician, various caregivers, the patient and their family
  • Supervising and providing clinical direction to the RN, LPN, Chaplain, MSW, Aide, Music Therapist, Massage Therapist and the Volunteer & Bereavement Coordinator to ensure quality and continuity of services provided
  • Meeting with all field staff on a regular basis to provide guidance and information related to specific issues
  • Maintaining adequate staffing levels to meet patient needs
  • Coordinating services to ensuring the availability and competency of staff to respond to agency referrals and provide assessment visits within 24 hours of referral
  • Coordinating weekend on-call services for field staff
Required Qualifications
  • Graduate of an accredited school of nursing
  • Current RN license in state
  • Two (2) or more years of RN experience
  • Complete required work daily
  • Cope with work involving ill, disabled, and terminal patients
  • Sit, stand, bend, lift, and move intermittently
  • Ability to lift at least 25lbs and bear the weight of an average adult effectively
  • Excellent observation, verbal and written communication skills
  • Problem solving skills and basic math skills
Desired Qualifications
  • Experience supervising employees
  • Bachelor’s degree in skilled nursing (BSN)
  • Hospice and/or Home Health experience
  • Familiarity with Home Care Home Base (HCHB)

Hours
  • Office hours: 8:00 AM – 5:00 PM, Monday through Friday
  • On call rotation
  • Weekend rotation
