POSITION: Clinical Supervisor for ACT Team HOURS: Full-time (9am – 5pm) LOCATION: ACT – 256 Washington Street, Mt. Vernon, NY SALARY: $60,000 - $70,000 (commensurate with experience and credentials) Position Summary: The Assertive Community Treatment (ACT) Program Clinical Supervisor is a critical leadership position at The Guidance Center of Westchester (TGCW). Established in 2018, TGCW’s ACT program is an evidenced-based practice that offers treatment, rehabilitation, and support services, using a person-centered, recovery-based approach, to individuals that have been diagnosed with serious mental illness (SMI). Typically, individuals served by ACT have a serious and persistent psychiatric disorder and a treatment history that has been characterized by frequent use of psychiatric hospitalization and emergency rooms, involvement with the criminal justice system, alcohol/substance abuse, and lack of engagement in traditional outpatient services. ACT services are provided by a mobile, trans-disciplinary team who meet at the program participant’s residence and in mutually agreed upon settings in the community where the participant feels comfortable. ACT supports recovery through a highly individualized approach that provides participants with the tools to obtain and maintain housing, employment, relationships and relief from symptoms and medication side effects. The nature and intensity of ACT services are developed through the person-centered service planning process and adjusted through the process of daily team meetings. The goal of ACT services is to assist individuals to achieve their personally meaningful goals and life roles. ACT integrates the principles of cultural competence, addressing the impact of discrimination/stigma, and inter-system collaboration into its service philosophy. An essential aspect of ACT is recognizing the importance of family, community-based, and faith-based supports. ACT services include:

  • outreach and linkages to community supports
  • assistance with medication management
  • care coordination and support attending appointments
  • educational and vocational support
  • integrated mental health, addiction, and medical treatment
  • housing assistance
  • family education and support
  • wellness skills development
  • peer support
Reporting to the ACT Team Program Director, the ideal candidate will have experience and a passion for working with individuals who are living with severe and persistent mental illness. They will also have experience supervising a multidisciplinary team and providing clinical supervision as well as program oversight. It will be important for this new team member to be proficient with computers and able to creatively engage program participants in their care in community settings, including individuals who may be homeless.
Responsibilities:

  • Contribute to the team spirit, positivity, and collaboration in care delivery to program participants.
  • The primary clinical voice/expert for case consultations on the team.
  • Serving as the secondary team leader for coverage in the absence of the program director/team leader
  • Providing individual clinical supervision for the peer specialist, vocational specialist, substance use specialist, and other specialists as the team grows.
  • Actively provide direct service, cross train staff, and provide clinical and Wellness Self-Management prospective into the day-to-day functioning of the program.
  • Providing clinical training and group supervision to the full ACT team as needed
  • Providing care to a small caseload of the roster, including documentation
  • Participate in morning team meetings
  • On-call rotation for the crisis coverage
  • Promote, support, and monitor staff development in evidence-based practices inclusive of the ACT model, ensuring that ACT integrates best practices and achieves its goals and objectives.
  • Supervise and/or conduct intakes and complete admissions documentation in accordance with guidelines.
  • Complete and/or ensure the completion of all documentation of contacts with program participants in electronic medical record and CAIRS system, adhering to regulatory compliance, clinical appropriateness, participant goal(s) and billing needs.
  • Assist ACT Team Director in review and sign off on documentation in electronic medical records for supervisees (and other team members when needed) to ensure regulatory compliance, clinical appropriateness, participant goal(s) and billing needs.
  • Assist ACT Team Director to develop/maintain protocols, workflows, and policy and procedure manuals to ensure that the program meets all regulations, guidelines, and quality assurance requirements.
  • Remain current on policies and procedures of Medicaid, managed care Medicaid, Medicare and insurance companies to facilitate coordination of program’s financial responsibilities.
  • Assist ACT Team Director to develop/maintain systems to monitor participant care on regular basis and proactively address concerns.
  • Lead team meetings and clinical case conferences; facilitate case presentations and conferences with other staff, family members of participants, other service providers, etc.
  • Spend approximately 80% of time providing direct services to participants individual or group onsite or offsite.
  • On-call and provides coverage as needed, especially related to crisis intervention.
  • Performs other duties as needed to ensure the success of the program.
Qualifications:
  • New York State Licensed Clinical Social Worker (LCSW or LCSW-R), LMSW, LMHC, LMF, or relevant credential.
  • Positive attitude and demeanor with an ability to engage individuals of diverse backgrounds, perspectives and needs.
  • Organized, solution-focused team player, with leadership qualities that support a collaborative and supportive interdisciplinary team approach.
  • Demonstrated passion and commitment to supporting the well-being and recovery process of individuals and families struggling with complex challenges related to severe and persistent mental illness and/or alcohol and substance use.
  • Ability to conceptualize and implement whole person care, integrated medical, mental health, addiction, and other social determinants of health needs.
  • At least 1-year clinical supervision experience of interns, peers, and/or other clinicians/counselors/nurses.
  • Experience with electronic medical record documentation.
  • CASAC-T or CASAC a plus; Bilingual Spanish speaking a plus.
