Lorian Health, a leading provider of home health care services for southern California, is seeking an experienced RN Clinical Manager for our San Diego, CA agency. This position is 50% in the office and 50% in the field hands on patient care.

Job Summary
The RN Clinical Manager supervises and coordinates staff to ensure provision of quality patient care and effective team performance to meet company goals in accordance with company’s philosophy, policies, mission, and vision, and prescribed local, state, and federal regulations.
Responsibilities for RN Clinical Manager (Riverside)
- Oversees and assures the delivery of all patient care and services according to professionally-accept standards of practice.
- Participates with the physician, referral sources and professional staff in decisions regarding patient eligibility and suitability for home health services
- Ensures that upon admission to LORIAN all patients are thoroughly assessed and that based on that assessment the appropriate clinical services are rendered to the patient .
- Coordinates regular case/team conferences and promotes communication of information relevant to the patient care process to ensure quality of care and coordination of services.
- Assesses clinical practices of specified disciplines through the review of electronic clinical records to ensure that standards of care are met and quality services are provided in accordance with prescribed LORIAN’s, local, state, and federal guidelines.
- Coordinates staffing patterns to optimize utilization of clinical staff while at the same time meeting patient needs and the needs of the interdisciplinary geographical clinical teams.
- Reviews intervention by different disciplines and minimizes duplication of effort and overlapping of services (i.e. exhaust OT intervention prior to referring HHA).
- Reviews patient’s address and assigns patient to clinicians respective of their territory.
- Works with the Human Resources Department in anticipating and recruiting clinicians in areas of need.
- Collects and analyzes staff visit data to ensure that individual clinicians are consistently meeting prescribed productivity standards.
- Collaborates with Quality Assurance Department in the dissemination and implementation of new regulatory and clinical guidelines.
- Completes, maintains, and submits accurate and relevant clinical notes regarding patients/clients condition and care given in accordance with LORIAN policies and procedures.
- Participates in in-service education programs and clinical meetings.
- Protects patients and employees by adhering to infection-control polices and protocols.
- Maintains confidential patient records in accordance with privacy and security standards of the Health Insurance Portability and Accountability Act (HIPAA) and other applicable state laws.
- Complies with accepted Professional standards & practices.
- Performs other related duties incidental to the work described herein.
Required Education and Experience
- A graduate of an accredited school of professional nursing.
- Currently licensed to practice as a RN in the state of CA.
- Two years of RN experience, home health care preferred.
- Two years in a supervisory position, preferred.
Additional Qualifications for RN Clinical Manager (Riverside)
- Possesses and maintains current CPR certification (BLS required).
- Is a Possess and maintains a valid driver’s license licensed driver.
- Possess and maintains an automobile that is insured in accordance with LORIAN requirements.
- Knowledge of Microsoft Office Systems.
- Working knowledge of State regulations (e.g. Title 22 (CA), or Joint Commission, and Federal Medicare Conditions of Participation regulations.
- Possesses and maintains good physical stamina and mental health. Presents a pre-employment physician’s health clearance, including negative TB skin tests and/or CXR and other tests as required by specific state statute.
- Has the Ability to read and follow written instructions and document care given in the patients’ electronic medical record.
- Ability to work with adults (18 years or older), demonstrating tact and patience.
- Is flexible and cooperative in fulfilling the role obligations.
- Knowledge and ability to apply community health principles and practices.
- Ability to supervise, guide, and develop skills and performance of service personnel.
- Ability to assist in evaluating personnel a minimum of one time per year.
Physical Requirements
- Is able to present a pre-employment physical which states they are free from health conditions which would interfere with the employee’s ability to perform assigned duties, and is free from signs or symptoms of infectious disease and is signed by the person who performed the assessment. A similar physical is required annually thereafter.
- Is able to pass a pre-employment Protein Purified Derivative (PPD) tuberculin skin test and annually thereafter. If unable to pass PPD testing or have history of positive PPD testing, a Chest X-ray will be required of a new employee and every five (5) year thereafter.
- Subject to driving, sitting, standing frequently.

Time Off: From date of hire, employee shall begin annual accrual of 10 vacation days, 6 sick days, and 9 holidays to be used as scheduled/needed.
