Now Hiring - Clinical Records Coordinator - North Wales, PA
Hours: M-F 8:30AM to 5:00PM
The Clinical Records Coordinator will be responsible: - Initiates the Clinical record at time of admission including initial preparation, interim maintenance, periodic tracking, closure and filing
- Ensures that: Chart is prepared with patient name and number, putting chart in designated file, tracking submission and receipt of plans of treatment, files the plans of treatment, histories and physicals, physicians orders, certification statements, and other documents as appropriate
- Assures that all documents are filed in patient charts on a weekly basis
- Files and maintains medical records according to company policy, state and federal requirements
- Assures that medical records removed from the department are logged in and out and that the record is returned that same day
- Orders any DME/HME requested by the Case Manager. Enters this data into the system
- Communicates any DME issues to the Regional Manager
- Prepares the activity report for data entry
- Recognizes and performs duties which need to be performed although not directly assigned
- Primary point of contact for all medical record activities between the office, IDT, facilities and physician
- Contacts the physician to insure that physician certifications are signed within 4 business days of admission Obtains missing or incomplete documents from the Regional manager or designee in a timely manner
- Copies any clinical information requested by staff members in a timely manner
- Abstracts information from records as authorized/required for insurance companies, Medicare, Medicaid, VA, ETC
- Retrieves and releases medical records/health information when requested by authorized personnel, in accordance with professional ethics and company procedures
- Through regular contact, supports staff efforts for achieving timely completion of their medical records
- Prepares paperwork for Team meeting, which includes but is not limited to: Updated Medication Profiles, Certs and Recerts, Team Census, Admissions, Discharges and Transfers
- Tracks and reports census for the team
- Listens to the daily morning report and assures that any changes of status, discharges, admissions or any other pertinent information is received by the On-call nurse or Case Manager
- Assures, via fax, physicians signature on recertification of Hospice benefits Recognizes and performs duties which need to be performed although not directly assigned
- Performs weekly clinical record audits, identifies missing or incomplete documents and immediately contacts the regional manager
Qualifications: Education: - High School Diploma Required
Professional Experience: - Experience with Medical Terminology and both Electronic and Paper Charts
- 1 year of related experience
