You can also contact us directly at: internshipprogram@kennethyoung.org The Adult Community Living Intern provides a wide array of services and offers internship opportunities providing community support and counseling services to adults with chronic mental health issues, and co-morbid complex medical conditions and substance use issues. Treatment services are provided on and off-site to include both group and individual mental health rehabilitation services to assist clients in achieving rehabilitative, resiliency and recovery goals. Services may include assessment, outreach and engagement, community support, case management, benefits access and restoration, crisis response, and therapy to focus on increasing an individual’s independent living skills, maintaining housing, and reducing the risk of the need for hospitalization or intensive day programming. Counselors are responsible for completing comprehensive mental health assessments, treatment plans, and providing a variety of services based on one’s treatment needs and recommendations. Various levels of support and intervention types are available based on an individual's needs and preferences, and may range from monthly to multiple times weekly. Services may occur in client homes, community settings, nursing and rehab facilities, hospitals, and in-office, and counselors learn how to coordinate with other members of a client's treatment team (including medical doctors, psychiatrist, family members, landlords, law enforcement, hospital staff, and other KYC service providers). Examples of Intensive Mental Health Outreach services include: Mobile Therapy, occurring in permanent supportive housing, nursing homes, and typical homes in the community; Community Support Team services, occurring across multiple teams and combining therapeutic support and case management skills to gain stability for chronic conditions; and Assertive Community Treatment, occurring through a team designed to assist with maintaining functioning in the community for individuals that may otherwise be institutionalized without frequent contact and assistance toward meeting needs.
