Working toward the goals of ending HIV as an epidemic in the United States and improving the long term health outcomes of people living with HIV, Housing Works provides a comprehensive range medical, behavioral health, vocational and support services to people living with HIV/AIDS as well as innovative prevention support for individuals at high risk for contracting HIV. The Be InTo Health project, a partnership between Housing Works and NYC DOHMH, is a specialized intervention aimed at helping HIV positive Black and Latina cisgender, transgender, non-binary and/or genderqueer women reengage in HIV care and other critical support services. Working as part of the interdisciplinary team that includes a Project Manager, Clinical Social and an RN, the Client Navigator will work with assist with the recruitment, enrollment and delivery of enhanced navigation services, including helping clients access support services, entitlements and benefits, transportation services, accompaniment to appointments and other service interventions that promote retention in care and activate clients in the management of their own healthcare. Ongoing training on the Be InTo Health intervention will be provided by Housing Works and DOHMH, however, the ideal candidate will have experience working with high risk populations, including but not limited to HIV, homeless, substance use and mental health. Additionally, candidates should have a high degree of cultural competency in working with cisgender, transgender, non-binary and or genderqueer Black and Latino women. This position is included in a bargaining unit of Housing Works’ New York City employees represented by a labor union known as the Retail Wholesale and Department Store Union (“RWDSU”). Accordingly, the RWDSU has the exclusive right to bargain over the terms and conditions of employment related to this position and this position may become covered by the terms of a collective bargaining agreement (a “CBA”) between Housing Works and the RWDSU.
