Nystrom & Associates, Ltd. is currently looking for a PART-TIME Certified Peer Support Specialist to provide peer support services to clients currently enrolled in in the Behavioral Health Home Program . The role of the Certified Peer Support Specialist is to provide ongoing encouragement and feedback to clients over the phone or in-person in relation to each client’s health and wellness. In addition, the Peer Support Specialist will:
Serve as a coach clients, helping clients discover their strengths
Interact with clients on the phone or in-person on a regular basis to build a trusting relationship
Assist clients with setting up and preparing for appointments and ensure clients are aware of mental health resources available to them
Provide clients with health promotion services by assisting them with skills related to overall health and wellness
Assist the Behavioral Health Home Systems Navigator and other team members in coordinating care
Provide health education to clients to increase their understanding of their health conditions and how these conditions affect their life
Identify barriers in clients’ care and work collaboratively with the client and other providers to minimize these barriers
Meet clients before/after appointments to provide encouragement, organize health concerns, and review provider feedback
Maintain legible and organized patient electronic health records in accordance with policy of the company and requirements of the State of Minnesota. Concurrent documentation is practiced.
Attend regularly scheduled staff and training meetings

To be eligible for this position, you must meet the qualifications of a Certified Peer Support Specialist 1 or 2 as defined under Mi nnesota Statute 245.462 subdivision 17. Please note that Nystrom & Associates does not provide peer support certification courses.
Certified Peer Support Specialist Level I must meet the following requirements:
Is at least 21 years of age
Has a high school diploma, GED or equivalent
Has a primary diagnosis of mental illness
Is a current or former consumer of mental health services
Successfully completed the DHS approved Certified Peer Specialist training and certification exam.
Certified Peer Specialist Level II – must meet all requirements of CPSS Level I and meet one or more of the following:
Is qualified as a mental health practitioner
Has at least 6,000 hours of supervised experience in the delivery of peer services to people with mental illness
Has at least 4,000 hours of supervised experience in the delivery of services to people with mental illness and an additional 2,000 hours of supervised experience in the delivery of peer services to persons with mental illness
Additional qualifications include:
Ability to work independently
Valid Driver’s License/ Auto Insurance/ Reliable Vehicle
Proficient in the use of computers
Previous experience using electronic health records preferred; comfortable utilizing electronic documentation concurrently
