JOB SUMMARY
At NorthBay Healthcare the Certified Diabetes Educator (“Educator”) is a licensed Clinical Psychologist, Registered Nurse, Occupational Therapist, Optometrist, Pharmacist, Physical Therapist, physician or Podiatrist who provides structured education to patients and their family’s related to diabetes. Primary teaching responsibilities will address self-management of diabetes. Diabetes self management education will include diabetes disease process and treatment options, nutritional management, physical activity, medications including insulin administration, monitoring blood glucose, prevention and treatment of chronic and acute complications, healthy coping and problem solving skills related to diabetes.
The Educator will also be responsible for overseeing daily operations of the Diabetes Self Management Education Program at NorthBay Healthcare. In partnership with the supervising physician, as appropriate, Educator will facilitate the diabetes program in identifying and executing a minimum of one Continuous Quality Improvement project annually. The Educator will plan an annual meeting of the NorthBay Diabetes Advisory Board and collaborate with the physician dyad to prepare all Board materials.
PRIMARY JOB DUTIES
- In collaboration with physician, as appropriate, design and present specific curriculum for diabetes education, insulin pump training and education, continuous glucose monitoring, and management of chronic disease.
- Ability to function independently in an expanded role as well as participating effectively within the team.
- Assess patient and family needs for intermittent education, providing content to meet specific individual needs on a timely basis.
- Document patient education including patient learning needs, responsiveness and action plans.
- Conduct Process Improvement projects annually to confirm the efficacy of the education provided.
- In concert with practice leadership and providers, evaluate additional patient education opportunities, developing and implementing appropriate programs.
- Research and maintain current, effective learning tools to expedite the integration of information by the patient.
- Oversees the planning, implementation and evaluation of the DSME program.
- Insure the Diabetes Advisory Board meets ADA and AADE guidelines and is actively involved in DSME program.
- Serves as liaison between staff, management, patients and Advisory Board.
- Advocates for patients with insurers, pharmacies and any other area to ensure patient care goals are achieved, as appropriate.
- Monitors and facilitates maintenance of staff qualification.
- Responsible for securing and maintaining ADA and AADE recognition and participating in the evaluation of the program’s effectiveness.
- Will maintain internal certification to train staff on Waived Testing for glucometer and Hemoglobin A1C testing.
- Interpersonal skills sufficient to counsel and instruct patients and families; to serve as a resource person for other health care professionals in the institution who are involved in the care of diabetic patients, and to conduct formal educational programs.
- Plans, develops and implements individual and group diabetes inpatient and outpatient education for adults, pediatrics, and their families and caregivers. Maintains in-depth knowledge and understanding of the nature of diabetes and related issues and patient care concerns.
- Collaborates with the client, significant others, health care providers and team members.
- Collects and analyzes specific client data and outcomes. Assists in the development and implementation of educational and dietary counseling interventions based on client-specific needs.
- Maintains clinical expertise in care related to diabetes management and other chronic diseases. Leads self-management training and education to type 1, type 2, and gestational diabetic patients, as well as colleagues.
- Participates in the assessment and development of diabetes education materials and programs.
- Participates in program quality and effectiveness evaluation activities. Participates in program marketing activities.
- Performs other duties as assigned.
Education: Minimum Associates level education required. Education will be equivalent to licensure required for CDE achievement.
Licensure/Credentials: Active certification as Diabetes Educator. Active California license as required by chosen clinical discipline.
Experience: Minimum of one years of experience functioning as a Certified Diabetes Educator with direct patient care delivery in an ambulatory care setting highly preferred. Program management experience preferred.
Skills: Must have knowledge of the skill necessary to perform patient and family education in a one on-one setting, as well as a classroom setting. Must be able to develop a curriculum, patient education materials and evaluation tool.
