Monday - Friday, 8:15am to 4:15pm

Qualifications for the Position:

  • B.S. in Psychology, VR or related Human services field; M.S. preferred with upper experience range.

  • One (1) to three (3) years prior adult counseling or related work with persons with disabilities.

  • Valid driver’s license with appropriate levels of insurance;

  • Eligible for C.R.C. and willingness to pursue; Already obtained CRC preferred

  • Required skills, knowledge and ability to perform job.

  • Ability to develop and maintain effective work relationships.

  • Ability to facilitate and coordinate the work of others.

  • Ability to learn new tasks quickly.

  • Effective oral, written and digital communication skills including mastery of Microsoft Office Suite

  • Knowledge of non-profit educational/training environment.

  • Familiarity with funders such as DVRS and DDD.

  • ASL/ Deaf Studies or bi-lingual preferred

    Summary Description of the Position:

    This position coordinates the implementation of Individual Employment Plans (IEPs) and Individual Service Plans (ISPs) . Assesses and monitors the implementation and effectiveness of clinical interventions. Conducts individual and group therapeutic sessions with program participants. Coordinates behavior management. Discusses areas affecting program participant with related health services, DDD case managers, DVR counselors, Support Coordinators and families/legal guardians/group home staff. Fosters input in communications. The Case Manager recognizes that the highest quality of service is delivered with compassion, respect, and hospitality, which are core values of the agency. Maintains confidentiality of departmental records, persons and activities.

    Is aware of HIPAA regarding Personal Health Information (PHI).

  • Be available online and by phone for the duration of their usual workday, minus breaks and rest periods
  • Respond promptly to communication via messaging app, email, and phone
  • Take all required break and rest periods, as if they were in the office
  • Communicate consistently regarding their workload and status (break, lunch, working on a project, etc.).
  • Completes projects within required timelines and keeps Supervisor up to date on completion.
  • Follow all Agency procedures and policies (including those that relate to Remote Services)
  • Refrain from using alcohol or illegal drugs

Essential Duties/Responsibilities of the Position:

  • Provides case management services for consumers and their families; provides vocational rehabilitation counseling on an as-needed basis.

  • Writes and implements rehab plans; recommends and tracks internal and external referrals; records significant information for case entries.

  • Monitors consumer progress through record keeping and communication with other community agencies (DVR, DDD, medical personnel, etc.).

  • Provides consumer referrals to job development.

  • Promotes and facilitates self-advocacy among consumers in cooperation with SMARTS (Self-Advocates are Ready to Speak Up).

  • Supports consumers in community employment transitions.

  • Complies with standards set forth in DVR, DDD contracts and CARF manual.

  • Observes agency and departmental procedures concerning infection control, safety, hazardous communication, emergencies, behavior management and professional conduct.

  • Monitors consumer areas for safety and ease of access.

  • Responds appropriately to interruptions, emergencies and various safety and evacuation drills.

  • Develops professional competencies; keeps abreast of related literature, enhances technical skills and knowledge.

  • Participates in departmental meetings, information sharing and in-service training. Offers ideas for improved services and operations.

  • Promotes teamwork. Participates in planning and implementing cross-training/cross utilization for various department functions.

  • Works flexible hours as needed, including evenings and weekends based on consumer needs.

  • Prepares and keeps charts up to date for DVRS audits by PPDS (Program Planning and Development Specialist).

  • Attends bi-weekly Staff meetings and Clinical Case Conferences to presents updates and develop IEPs (Individual Employment Plans) including, but not limited to, reporting on client production rates and earnings/calculations.

  • Reviews referral profiles and maintains appropriate paperwork including case record documentation and billing reports

  • Coordinates with the VRAS Program Supervisor and Finance Department on consumer payroll, billing, ordering of supplies, client timesheets, piece rates, time studies and prevailing wage rates. Ensure subminimum wages are updated regularly as required.

  • Performs other duties as assigned.
