Classification: Non-exempt Location: Claremont Section, Bronx, NY Position Type: Case Manager We are seeking an experienced Case Manager for Lantern's Supportive Housing in Bronx, NY serving formerly homeless individuals living with chronic physical illness and/or mental health issues. The Case Manager will be expected to assist this population in stabilizing their lives, maintaining their housing stability and attaining their physical, emotional, vocational, and social goals. Case Managers are responsible for assessing the needs of the client, linking clients to services, planning a course of action towards stabilization and re-housing. They will also be monitoring progress, and documenting client contacts. Additionally, the Case Manager will be passionate about helping people; be a proactive and creative problem solver; and work well both as a part of an interdisciplinary team and independently. While performing the duties of this job, the employee is regularly required to stand, walk, sit, stoop, climb stairs and/ or kneel. Provide integrated counseling and case management services that effectively engage residents and promotes health and wellness, personal growth, and housing stability.
Required Education and Experience:
BA/BS required, preferably in a related field such as social work, sociology or psychology.
Work Environment: This job operates in a supportive housing site and works with a population who experiences mental illness, addiction, disability, HIV, and other chronic illnesses.
