The case manager is responsible for tracking every youth on their caseload and assuring that every youth has all the required documentation needed to be ARC compliant. An ARC case manager is responsible for assuring every youth on their caseload is made aware of the services at Hearts and are offered what they need. An ARC case manager is responsible for all paperwork for each youth on their caseload as indicated in the attached ARC procedures. Responsibilities · Completing weekly goals assigned by associate director. · Actively following the youth assigned to your caseload and offering them the services they are needed so they can meet their goals set forth in their ISPs. · Uploading counseling notes on VOS and Database as determined by the associated director, either weekly or monthly. · Completing ISPs quarterly on paper and having youth sign as well as entering the ISPs onto the VOS. · Keeping up with youth progress and knowing when they have met their goals so they can be exited and moved into follow-up. · Completing counseling note report on time and sending it to your associate director. · Completing exit forms with the proof when a youth is ready to exit and sending to the associate director. · Keeping track of when a youth on your caseload turns 18 and obtaining proof of their Selective Service if applicable, and making certain Immigration Form is signed. · Making sure that the youth reaches their L/N requirement. · Responsible for being knowledgeable about services that can be provided to the youth. This means knowing what is offered by Hearts and being able to make referrals to other programs if needed. · Performs other duties as assigned. Requirements and Skills · Communication Skills: Case managers need to talk with clients and understand their needs. You will also need to speak with others on your client's behalf and act as an advocate for them. · Compassion: Case managers will be working with people who are sometimes in dire situations or going through difficult transitions. · Time-Management and Organizational Skills: Case managers have multiple clients with various needs and are in various stages of need. There is a lot of paperwork that will need to be filled out and you will be working with government officials, as well as other workers. You will need to be organized and efficient with your time. · Problem-Solving Skills: Clients are looking for someone to help them find solutions to their problems. You will need to think critically and clearly about their situations to find solutions.
