Senior Helpers is looking for an experienced and dedicated Care Liaison to join our team. Senior Helpers is a non-skilled home care company providing services throughout Chester, Delaware and Montgomery Counties.
The Care Liaison will have the overall responsibility of managing and developing all client services and business development for Senior Helpers within the community. Goals are but not limited to: retention of clients, managing the growth of hours from current clients, building the business within the community, implementing policies and process, education of programs Senior Helpers offers to facilities, positive customer service of the quality of services provided and having a positive impact on the morale of office staff.
Primary Responsibilities (including, but not limited to):
- Liaison between internal staff members and clients. Responsible for ensuring staff communication is accurate and complete.
- Complete in-person care assessments and contracts for clients joining Senior Helpers.
- Complete reassessments as needed based on the clients’ needs.
- Develop client care plans with detailed information for care staff.
- Communicate and network with all employees on-site in the Senior Living Communities.
- Oversee functions of scheduling to ensure the client's needs are being met by the scheduling department.
- Complete documentation of all interactions related to clients, referral sources and caregivers in a timely fashion.
- Communicate with providers such as skilled home care, private care managers, and hospice, etc. as needed to ensure care is communicated effectively.
- Ability to identify the needs of clients and make referrals to other providers.
- Responsible for potential client’s inquiry process.
- Represent Senior Helpers at networking events.
- Participate in the On-call rotation
- Other duties as assigned.
Qualifications
- Health care background required
- Networking experience required
- Management experience required
- Bachelor’s degree preferred
- Previous home care experience preferred
- Three years of related work experience strongly preferred
- Must have exceptional organizational skills, attention to detail, and the ability to prioritize in a changing environment
- Must have excellent verbal communication skills and follow-up skills with prospects, clients, and client families
- Ability to build rapport quickly
- Creative, innovative, and problem-solving ability
- Proficiency in Microsoft Word, Excel and Outlook required
- Ability to work independently and as part of a team
