GENERAL OVERVIEW OF KEY ROLES & RESPONSIBILITIES: The Care Coordinator, meeting the requirements of a Case Manager, will provide intensive care coordination to children/adolescents and their families. They will assure collaboration of all outside services as well as securing new services that are needed. ESSENTIAL & CORE FUNCTIONS: 1. Provides coordination of all services for IBHS consumers and their families well as linkages to new services needed. 2. Ensures appropriate referrals to community resources and follow-up on these referrals. 3. Ensures the timely completion and submission of authorization packets. 4. Ensures the timely completion and submission intake packets to A&E center same day of packet submission. 5. Consults with and documents contact with IBHS consumers’ unique caregivers, at minimum bi-weekly. 6. Executes discharge plan by making all referral to community resources. 7. Schedules and co-facilitates all interagency meetings. 8. Schedules all discharge meetings. 9. Obtains and updates yearly all consents. 10. Provides mental health interventions to a range of consumers. 11. Monitors consumers’ behaviors emphasizing prevention rather than intervention. 12. Assists teacher and/or unique caregiver with implementation of behavioral plan. 13. Supervises/monitors activities of child/adolescent in accordance with the behavioral plan with the emphasis on prevention rather than intervention. 14. Intervenes as needed when problem behaviors are exhibited, which includes but are not limited to counseling, removing them from the area, verbal prompts, securing the assistance of clinical staff, etc. 15. Prevents crisis situation however when necessary deescalate children in crisis. 16. Assists Lead Clinician with groups. 17. Inputs outcome data into computer system. 18. Collects outcome data for consumers. 19. Follows up on all consumers needing hospitalization. 20. Completes follow up on discharged IBHS consumers. 21. Obtains all authorization dates and numbers. 22. Maintains current list of viable referrals for IBHS program. 23. Follows up on all consumers needing hospitalization. 24. Collects information surrounding referrals including but not limited to CSAP paperwork, treatment history, school behaviors, etc. 25. Identifies and establishes contact with community resources. 26. Completes all paperwork within specified time frames. 27. Maintains Psychologist schedule and obtains information needed for these evaluations. 28. Follows up on all evaluations that recommend outside services. 29. Performs assigned duties during the summer months when school is not in session. 30. Completes home and community visits. 31. Represents program/Agency at community activities/fairs. 32. Adheres to WES’s Code of Ethics and comply with State Mental Health Code. 33. Attends trainings as required by WES. 34. Participates in continuous quality assurance/program development. 35. Complies with WES standards for service delivery. ADDITIONAL RESPONSIBILITIES: 1. Performs other duties and special projects as assigned. PREREQUISITES & QUALIFICATIONS FOR THE POSITION: 1. Bachelors Degree from an accredited university in a mental health discipline such as Psychology, Counseling, Clinical Social Work, or Marriage and Family therapy preferred. 2. One (1) year verified experience in case management or counseling with children and families, or is a licensed registered nurse with one (1) year experience. 3. Valid FBI clearance, criminal history check and child abuse history clearance required. 4. Participation in and completion of all CBH mandatory in-services (must be completed with three (3) months of hire and then annually from the date of the initial training).
