POSITION SUMMARY

The Parent Navigator is an important member of the Community Support for Families team and provides support, empowerment and advocacy assisting families with information, education, and linkage to community resources. The Parent Navigator provides peer support and mentoring to families to assist them in successfully navigating the service system. The Parent Navigator functions as part of a team, coordinating with other program staff to build family engagement and to help families accomplish their goals’. Parent Navigators deliver services in clients’ home or community-based settings depending on the needs of the families.


EDUCATION AND EXPERIENCE/QUALIFICATIONS

High School Education or GED Equivalent plus 3 years of experience working with families in the community are required. Basic computer skills and ability to communicate effectively verbally and in writing is also required.


Current program needs require the ability to fluently speak English and Spanish.


SCHEDULE

Part time, 20 hours per week

ESSENTIAL DUTIES AND RESPONSIBILITIES

Provides empowerment and advocacy for families, focusing on assisting families in developing effective partnerships with schools, providers and others to best meet their children and family’s needs.

Provides peer support and role modeling to teach families how to identify needed resources and supports in their communities.

Adheres to practice standards/protocols of service model and of agency.

Supports direct case management services and supports the development of case management plans specific to each assigned client and family, including interventions necessary to increase the probability of treatment completion, and to improve life functioning.

Maintains records of direct services for reporting and tracking purposes and completes all documentation for client record in accordance with documentation timelines and in compliance with Medicaid, licensing and other credentialing authorities.

Attends team meetings, staff meetings or rounds to assist in the development of case management needs and service plan review.

Supports daily program specific operational functions as required. May transport clients to appointments or linkages as appropriate.

Provides service in the office and/or community settings outside of the office (e.g., homes, schools, hospitals, institutions, other community locations) as defined by the program.

Collects and enters data as defined by program.

Develops and maintains positive relationships with community partners, providers, and other stakeholders.

Supports implementation of safety planning with appropriate supervision and as defined by program.

Continues to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served as well as uses appropriate methodological approaches, skills, and techniques that reflect an understanding of culture.
