We are seeking a Behavioral Health Specialist (LCSW/LMFT) - Maternal Mental Health who will be responsible for evaluating and treating mothers and expectant mothers who are experiencing depression, anxiety and other mental health conditions. This position works primarily where women receive their maternal care and act as a regular team member by providing brief assessment, intervention and onsite consultation to the care teams regarding psychiatric/psychosocial concerns and health behavior change. The Behavioral Health Specialist for Maternal Mental Health works with individuals, groups, and families, providing evidence-based behavioral interventions for both mental and physical health conditions.

In this position you will:

  • Provide culturally competent behavioral health services that meet the needs of clinic population
  • Deliver upon the service expectations of both our patients and fellow staff members by listening to their needs; engaging in positive interactions; and following through on promises made in a thoughtful, efficient, timely and courteous manner so that their total outcome is better than expected
  • Respect the dignity, confidentiality and privacy of patients
  • Work in a safe manner, adhering to general safety precautions and standards. Report any unsafe conditions to their supervisor and/or the safety hotline
  • Develop and implement treatment plans for patients of all ages providing individual, group and family psychotherapy, as clinically indicated
  • Conduct accurate assessments and involves patients and families in developing achievable and acceptable treatment plans
  • Use treatment modalities aligned with brief interventions; including Motivational Interviewing, Behavioral Activation, Problem Solving Therapy, Harm Reduction, Dialectical Behavioral Therapy and Cognitive Behavioral Therapy
  • Using best practices, teach patients skills to manage chronic conditions
  • Triage and refer patients to appropriate level of care for behavioral health services throughout the community
  • Perform risk assessment and crisis management, including evaluation and referral to Emergency Services for patients who are experiencing suicidal and/or homicidal ideation, as well intimate partner violence
  • Facilitate referral to hospital or chemical dependency services where indicated
  • Utilize evidence-based clinical tools for tracking clinical outcomes in maternal primary care (e.g EPDS,PHQ9,GAD7)
  • Evaluate patients using appropriate DSM IV and DSM V criteria
  • Demonstrate effective practice management skills
  • Stay on time when conducting consecutive appointments with patients
  • Use continuity visits with patients
  • Appropriately refer patients to groups and classes (e.g. Postpartum Wellness)
  • Act as a key behavioral health provider by providing consultation services to both providers and patients
  • Advise and consults as a resource to physicians and staff
  • Provide brief consultative interventions to individuals and family members in person and over secure telephonic and video systems
  • Clarify mental health diagnoses when needed
  • Assertively follow up with the Obstetricians-Gynecologists, Pediatricians, Primary Care Physicians, when indicated
  • Describe interventions in transparent language to encourage maternal primary care team support
  • Maintain a high degree of professionalism in documenting clinical encounters in electronic medical record system
  • Maintain appropriate patient records according to quality guidelines established by Providence St. Joseph and applicable accreditingnorganization standards
  • Write clear, concise chart notes
  • Prepare reports for patient records and summary reports on service delivery
  • Routinely complete all billing documentation
  • Serve as a supportive team member
  • Participate as an active member of the multi-disciplinary team within the clinic, providing team-based care; fully integrated with primary care health services
  • Participate in shared treatment sessions with providers and patients
  • Participate in integrative care team meetings and huddles to coordinate care
  • Readily provide unscheduled services when needed
  • Interface with community mental health and substance abuse treatment centers to coordinate care as appropriate
  • Participate in and lead therapeutic groups and activities and functions as an active member of the team
  • When possible and appropriate, take the initiative to offer help to co-workers in overload situations
  • Work at maintaining a good rapport and a cooperative working relationship with all staff and colleagues

Qualifications:

Required qualifications for this position include:

  • License for independent clinical practice in the State of California (or in the process of acquiring licensure in the State of California) and Licensed to practice independently as a Psychologist (PhD or PsyD), Social Worker (LCSW) or Marriage and Family Therapy (LMFT)
  • Current clinical licensure in the State of California required: Psychologist (PhD or PsyD) or Social Worker (LCSW) or Marriage and Family Therapist (LMFT)
  • 2 years of clinical experience in the provision of behavioral health services for Maternal Mental Health issues to a wide range of patients. Extensive experience treating patients of different cultural and socioeconomic backgrounds. A preferred skill is providing clinical services in Spanish language to meet the needs of the local Spanish speaking community
  • Must have excellent computer skills, able to work in an electronic health record and have proficiency with MS Office: Outlook, Internet, Word and Excel
  • The individual in this position must be comfortable with a flexible schedule, able to accommodate multiple and varied length patient visits per day, complete at least five to six patient visits daily, and adept in working closely with Obstetricians-Gynecologists, Primary Care Physicians, Psychiatrists and other team members. Must have a basic nontechnical knowledge of psychotropics and other relevant medications, and a basic understanding of female reproductive health as well treating Postpartum Mood Disorders.
