Employment Type : Full-Time

MUST HAVE A BACHELOR'S DEGREE IN THE HUMAN SERVICES FIELD, ONE YEAR EXPERIENCE, AND CURRENT QMHP-A/T CERTIFICATION.

Works as part of the Intensive Community Treatment (ICT) team to assure services and resources are coordinated among providers as needed to help individuals served live and function optimally in the community. Responsible for carrying out rehabilitation and support functions including but not limited to service coordination, education and skill building for recovery and mental health stability, as well as community resource linking and coordination.

Responsibilities:

1. Actively participates in daily ICT team meetings for client treatment planning needs.

2. Submits all administrative paperwork and completes all contact progress notes, assessments, as applicable, Individual Service Plans, quarterly reviews and other required documentation in a timely and accurate manner.

3. Advocates for services on behalf of individuals and their families/care givers as needed. Reviews human rights and responsibilities with individuals and/or guardians as outlined in ICT policies and procedures. Documents and reports suspected violations of client’s rights immediately as required by policy.

4. Consults with community agencies and families as appropriate to maintain coordination of the treatment process; links clients to community self-help programs and support groups as needed.

5. Assists clients with activities of daily living tasks including but not limited to money management/budgeting, utilization of community resources, household tasks, housing issues, personal hygiene, nutrition education, and management of physical health issues.

6. Provides individual support and education for skill development including but not limited to problem solving, symptom management; social skills, self-esteem and assertiveness training.

7. Provide support, education and encouragement to help clients socialize with peer group and families while addressing mental illness issues that prevent positive interpersonal interactions; organize and lead individual and group social/recreational activities to educate and increase social experiences, and provide opportunities for clients to practice social skills.

8. Treats all communications, written or verbal, with respect and confidentiality, and complies with HIPAA standards and regulations

Knowledge, Skills and Abilities:

1. Demonstrated knowledge of serious and persistent mental illness, intellectual disabilities, and substance use disorders.

2. Knowledge of assessments and individual service plans and their uses in service planning; case management practices and how to access and utilize community resources.

3. Ability and willingness to work with individuals with co-occurring serious mental illness, intellectual disabilities, and substance abuse disorders and their families/care givers, and other agencies.

4. Knowledge of treatment modalities and intervention techniques, such as behavioral supports, independent living skills, vocational support and training, supportive counseling, family education, discharge planning and service coordination.

5. Knowledge of the Department of Behavioral Health and Developmental Services

(DBHDS) regulations for providers of mental health services.

6. Knowledge of relevant Medicaid guidelines.

7. Demonstrated ability to communicate clearly and concisely both written and verbal, and working knowledge of computers and word processing.

8. Ability to complete all client assessments, as applicable, progress notes, individual service plans and quarterly reviews meeting quality standards.

9. Basic knowledge of use of medications in the care of individuals served.

10. Initiative, resourcefulness, and ability to work independently as well as part of a team.

11. Possession of a valid driver's license and an acceptable driving record.

12. Acceptable background and central registry check; acceptable drug screen.

Education and Experience:

Bachelor’s degree in social work, psychology, or human services related field with at least one year applicable related experience working with individuals with co-occurring serious mental illness, intellectual disabilities, and substance abuse disorders, preferably in community setting.

Must have a valid Qualified Mental Health Professional-Adult certification from the Virginia Department of Health Professions.
