Premier Long Term Skilled Nursing Facility located in Erie County, is seeking an Admissions Intake Coordinator/ Conceriege. The admission intake coordinator will be is responsible for the admission process for new patients by ensuring availability of reimbursement for services, identifying the clinical team to deliver care, and scheduling all patient visits within prescribed discipline and visit frequency requirements. The admissions intake coordinator will ensure communication needs of residents, patients and family members are met. He /She will provide care that reflects initiative and responsibility indicative of professional expectations. A sincere respect for the elderly members of society is a must as the position requires in depth communication with residents

Duties and responsibilities may include but are not limited to:

  • Assist in interviewing residents/guardians and obtaining required information and signatures.
  • Assist in the orientation program for residents/personnel in accordance with our established policies and procedures.
  • Receives referrals from physicians and facilities and initiates the intake process.
  • Admit and prepare identification records for residents in accordance with established policies and procedures.
  • Admit, transfer, and discharge residents in accordance with established policies and procedures.
  • Assist in recording all incidents/accidents. File in accordance with established policies and procedures.
  • Obtain the resident/guardian’s signature on all required permits, releases, authorizations, etc.
  • Maintain a current listing of all residents, their level of care, and assigned room numbers. Provide copies of such listing to nursing service, administrative personnel, government agencies and personnel, etc., as necessary.
  • Keep abreast of current federal and state regulations, pertaining to Medicare, Medicaid and Long-Term Care, and make recommendations on changes in policies and procedures to the Administrator.
  • Assist in developing and implementing policies for identification of financial resources of the resident.
  • Participate in community planning related to the interests of the facility and the services and needs of the resident and family.
  • Perform administrative requirements, such as completing forms, reports, etc., and submitting such to Administrator as required.
  • Schedules all visits to ensure that regulatory deadlines are met and staffing resources are optimized.

Qualifications:

  • Minimum Associates Degree preferred
  • Minimum 2-5 years’ experience as a successful Admissions Coordinator in a short term rehabilitation/long term care facility preferred
  • Healthcare, sales, marketing experience preferred
  • Knowledge of Microsoft Office, typing, medical terminology knowledge and data entry required
  • Excellent interpersonal skills; possess the ability to deal tactfully with residents, family members, visitors.
