The Patient Navigator will assist patients and their families through the cancer care continuum at Mount Sinai Hospital. The purpose of the Patient Navigator is to intervene with appropriate services and assistance to reduce the amount of time in delivering services in relation to diagnosis and treatment. The Patient Navigator will identify barriers to quality care and plan path for patients to access assistance.

MINIMUM EDUCATION:

  • Bachelor’s Degree in a health science or related field

MINIMUM WORK EXPERIENCE:

  • Effective Oral and Written Communication Skills
  • Proficiency in Microsoft Word, Access, and Excel
  • 1-2 years work experience in a community-based organization, preferably in a field related to child immunization or maternal and child health
  • Ability and desire to work with an array of stakeholders, including community residents, parents, ministers, clinic staff, and physicians
  • Sharp attention to detail
  • Experience in program planning and development
  • Some supervisory experience
  • Data coding, cleaning, and analysis experience a plus
  • Experience with the TOTS data system a plus
  • Proficiency in Spanish a plus
