About the roleHere is your opportunity to engage with the vulnerable members of your community and support them to access services within the community to enhance and improve their lives. In this role, you will have a client list of both Home Care Package recipients and NDIS participants and will help them navigate the systems and engage services to ensure their safety and promote re-ablement and independence. You will work with regional providers to ascertain their available services and engage with participants to consider innovative ideas to support their independence.
We are looking for
- An experienced Case Manager or Support Coordinator.
- A person who has local knowledge of their regions providers and services available.
- Someone who is passionate about maintaining independence in rural communities.
- A person with great communication and listening skills

Selection Criteria
- Essential : Commitment to the GV Health Values of Compassion, Respect, Excellence, Accountability, Teamwork and Ethical Behaviour and ability to exhibit behaviour which reflects our values.
- Essential : A Tertiary qualification in an appropriate Health and/or Human Services field, or extensive relevant experience
- Essential : Demonstrated ability to negotiate access to appropriate services in line with funding guidelines
- Essential : Well-developed interpersonal and communication skills, including the ability to initiate and maintain contacts with a broad range of service providers and community agencies.
- Essential : Knowledge of the Case Management approach to service delivery, in a rural community setting.
- Essential : A sensitive approach to working with individuals and a philosophy of empowerment.
- Essential : Sound experience of the knowledge and skills required in working with frail older people, people with disabilities and their carers.
- Essential : Satisfactory National Criminal History Record Check, Victorian Employee Working with Children Check, clear Disability Workers Exclusion Scheme List Check and completion of Commonwealth of Australia Statutory Declaration prior to commencement of employment
