Job Description

ADULT CASE MANAGER
Lebanon, New Hampshire

At West Central Behavioral Health, you will become part of a close-knit group of dedicated professionals working towards improving the lives of their clients and the community as a whole. You will work closely with the psychiatrist and the clients’ therapist to develop a treatment plan to assist clients in managing daily problems and stresses, developing new life structure, as well as cultivating life and illness management skills. Case management services are provided both in the office setting and in the community where you may assist the client in meaningful activities. Collaborate with other community agencies to coordinate services that will enhance a comprehensive treatment approach.
Your work will directly impact the lives of adults living with mental illness and their families through case management, skills training, psycho-education and professional support.
Position Requirements include:

  • BA/BS degree in social work, rehabilitation, psychology, education or a related human services field; or
  • Associate's degree in social work, rehabilitation, psychology, education or a related human services field and the following experience:
  • 2 years of experience working with persons who have severe mental disability or SED; or
  • 2 years of experience that provides a person with an understanding of mental illness and that was acquired as an adult in the provision of significant supports to persons with mental illness, including the experience acquired by family members of person with mental illness or by other person who have personal knowledge of mental illness.
  • Case management experience preferred, but willing to train
  • Excellent organizational, communication and customer service skills
  • Possess working knowledge of computers and Microsoft Office
  • Valid driver’s license, plus reliable and insured vehicle
