Job Description
Overview: Admission RN Friday - Monday 11:00am - 9:00pmResponsibilities: ESSENTIAL DUTIES:
  • Models a service attitude towards others, takes ownership in solving problems and takes the initiative to make things better, is friendly and courteous to residents, patients and co-workers
  • Assume primary responsibility for a patient admission including assessment if needed, calls report of admission to team
  • Actively participate in coordination of all aspects of patient’s care, in accordance with current professional standards and practice
  • Initiate communication with attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family
  • Practices Universal Precautions according to OSHA regulations and Faith Hospice policy
  • Develop and evaluate the plan of care
  • Participate in QAPI program and hospice sponsored in-service training
  • Participate in ongoing interdisciplinary comprehensive assessment
  • Maintains confidentiality of all patient information, sharing only what needs to be shared with the interdisciplinary team in order for the patient to be adequately cared for
  • Maintain regular communication with the Director of Business Development, Director of Clinical Operations and Patient Care Manager in order to follow-up for every admission.
  • Respects patient/family rights by: (knocking on doors before entering the residence, addresses patient/family by name, allows patient to make own decisions when appropriate)
  • Provides emotional support and compassionate care to the patient and family. Demonstrates listening skills, offers encouragement when appropriate, adjusts care to the patients physical condition each visit
  • Obtain data on physical, psychological, social and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care.
  • Seek input from other team members regarding the patient plan of care to obtain additional knowledge and support.
  • Maintain up-to-date legible patient records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur.
  • Instruct/Educate primary caregivers, volunteers, and employed caregivers to provide care as indicated; Contribute to patient and family counseling and education
  • Is prompt for admission appointment, calls if late or unable to keep appointment, keeping Admission’s Coordinator informed also.
  • Provides hospice 101 as needed, assists patient/family signing legal paperwork for hospice admission, completes admission process and nursing assessment and reports to Admission’s Coordinator upon completion of admission. Leaves report via VM for team
  • Informs the Patient Care Manager of unusual or potentially problematic patient/family issues.
  • Assist other nurse/case managers including assuming primary responsibility for patients when necessary
  • Attends and participates in orientation and appropriate in-services and staff meetings to improve knowledge and skills and to maintain competency.
  • Provide appropriate support at time of death. Reports death to appropriate physician.
  • Plans the administration of and/or administers medication with knowledge of actions, contraindications, side effects and desired outcomes.
  • Participates in agency and community programs as requested
  • Is available for holiday, on-call, illness and vacation coverage as needed.
  • Other duties as assigned by the Patient Care Manager.
Qualifications: MINIMUM QUALIFICATIONS:
  • Possession of a valid Texas Registered Nurse License
  • Proficient ability to communicate orally and in writing in the English language
  • Minimum of two years of varied work experience as a professional nurse and preferably one of the two years in an hospice setting
  • Valid Texas Driver’s License and reliable method of transportation
  • Ability to work independently
  • Ability to adjust to changing conditions and frequent demands
  • Ability to acquire new knowledge to perform additional or more difficult tasks
  • Ability to maintain a positive, stable disposition in performing, working in association with staff, patients and families
  • Ability to maintain a high level of performance in an environment of frequent demands and changes
  • Ability to meet new and/or stressful situations with the appropriate responses, ability to work effectively as part of a healthcare team, to share information freely, to use and seek supervision and consultation verbally and in writing, and to be able to follow both verbal and written instructions
